Inhaled corticosteroids in COPD

چکیده
The importance of presenting absolute cell numbers when counting cells in biological samples is illustrated by the potentially misleading interpretation of data in the paper by Marco Confalonieri and colleagues. The authors concluded that, in addition to reduced sputum neutrophilia, the number of sputum macrophages increased significantly following treatment with inhaled beclomethasone dipropionate in patients with COPD. However, the observed increase in the proportion of sputum macrophages from 19.6% before treatment to 35.8% following treatment is entirely attributable to the reduced number of sputum neutrophils. From the data presented in the paper, the absolute numbers of diVerent cells in the sputum can be calculated (table 1), revealing that the absolute sputum macrophage count was essentially unchanged following treatment. It is important that the absolute numbers of cells, and not simply their proportions, are presented when measuring diVerential cell counts in sputum or any other biological sample.

Efficacy and safety of inhaler steroids in COPD patients: Systematic review and meta-analysis of randomized placebo-controlled trials
Background: Chronic obstructive pulmonary disease (COPD) is a major health dilemma and cause of morbidity and mortality in either industrialized or developing countries and inhaled corticosteroids are widely used worldwide in these patients. In this systematic review, we aimed to analyze the effectiveness of these gents compared to placebo. Methods: Pubmed and Google Scholar literature search ...
متن کاملInhaled corticosteroids and hospitalisation due to exacerbation of COPD.
Previous studies have provided conflicting evidence as to the possible benefits of inhaled corticosteroids in the treatment of chronic obstructive pulmonary disease (COPD). Using the Saskatchewan healthcare databases subjects were identified who were aged > or = 55 yrs, initiating regular treatment for COPD but without any prior treatment for asthma. In the current nested case-control analysis,...
متن کاملDo patients with COPD benefit from treatment with inhaled corticosteroids?
In the past few years, treatment with inhaled corticosteroids has become increasingly important in asthma [1, 2]. It appears that inflammation of the airway wall is a major pathophysiological mechanism underlying asthma [3], and perhaps also chronic obstructive pulmonary disease (COPD) [4]. Some long-term studies in asthmatic patients over 1 and 2 yrs have shown that maintenance treatment with ...
متن کاملAnalysis of inhaled corticosteroid and oral theophylline use among patients with stable COPD from 1987 to 1995.
STUDY OBJECTIVE To document temporal usage trends for commonly used respiratory medications in patients with COPD. DESIGN We retrospectively evaluated baseline concomitant medications of 3,720 patients with COPD enrolled in 10 bronchodilator clinical trials from 1987 to 1995. The proportion of patients in each trial using inhaled corticosteroids, inhaled beta-adrenergics, inhaled anticholiner...
متن کاملEffects of inhaled corticosteroids on sputum cell counts in stable chronic obstructive pulmonary disease: a systematic review and a meta-analysis
BACKGROUND Whether inhaled corticosteroids suppress airway inflammation in chronic obstructive pulmonary disease (COPD) remains controversial. We sought to determine the effects of inhaled corticosteroids on sputum indices of inflammation in stable COPD. METHODS We searched MEDLINE, EMBASE, CINAHL, and the Cochrane Databases for randomized, controlled clinical trials that used induced sputum ...
متن کاملInhaled corticosteroids in COPD
C hronic obstructive pulmonary disease (COPD) is arguably the most common chronic disease of the lungs at present and, by 2020, it will be the third leading cause of death worldwide. COPD is associated with a relentless decline in forced expiratory volume in 1 second (FEV1) and, in the later stages, the condition progresses to pulmonary hypertension and hypoxic respiratory failure. Few interven...
